We're sorry to see you have chosen to end your Plan Management service with Skylight Mental Health.

Please complete the form below so we can begin organising the conclusion of your Plan Management service.

We recommend seven day's notice period so we can give your providers enough notice to submit outstanding invoices to us, as your new Plan Manager may refuse to process invoices for services delivered while Skylight Mental Health were your Plan Manager. Skylight Mental Health will not be able to process any invoices after the date entered.
We require this information in case we need to pass invoices to your new Plan Manager if they are sent to us after your service end date.
