This online Service Request form is a new feature at Skylight, and we are eager to hear your feedback. After completing the form, you will have the opportunity to complete a short survey, or alternatively a member of our Customer Relationship Team may give you a call to request feedback. We appreciate any thoughts or feedback you might have.

Before you start, make sure you have your NDIS number, and your contact information, including your phone number and email address, at hand. Also, have a clear idea of the services you wish to access through NDIS. If you’re uncertain about which services would best suit your needs, we encourage you to submit a general enquiry here. Our team will connect with you to explore your options and provide tailored advice.

What Happens Next?:
By completing and submitting this form, you are formally requesting NDIS supports from Skylight Mental Health. Once received, our team will review your application to determine your eligibility. If your request is approved, we will finalize your service agreement and send it to you for confirmation. Additionally, the necessary NDIS funding will be allocated from your current plan as applicable.
