Community Connections Program
Support to stay connected
The Community Connections Program supports people who are not eligible for mainstream programs such as the NDIS or My Aged Care to become more involved in their community and connect with local support networks and services.
This is a short-term program, delivered over up to 12 weeks through a statewide network of partner organisations. Support focuses on helping each person establish and maintain meaningful social connections, strengthen community participation, and build a sense of belonging.
The program works alongside people to identify what connection looks like for them and supports pathways that can be sustained beyond the program.
Who Can Access the Program
The Community Connections Program is available for people who:
Live in South Australia
Want to strengthen connections with community, social networks and services
Are aged 18 to 64
Are not receiving support through NDIS or My Aged Care
Our Community Connections Team Will:
Work with you towards your personal goals
Link you with local services and supports
The team can support you to connect with Counselling, Financial services and Wellbeing Activities
Help you to explore new opportunities
We can support you to find new opportunities such as volunteering, training and local community activities.

Lived Experience Telephone Support
A service available to people living in the Adelaide metropolitan area. Reach out to LETSS between 5pm and 11:30pm, 365 days a year, and experience the support and understanding that comes from those who have walked a similar path.
Country Wellness Connections
This program provides mental health services to rural South Australia. Skylight’s Country Wellness Connections provides support and mental health services to people living in the Murraylands and Limestone coast.
Wellness Connect
Wellness Connect offers one-to-one peer coaching that supports recovery for people living with mental health challenges. Support can focus on receiving practical help with daily living, looking after your health, finding a home, gaining employment.
