Get support, get connected. Our Individual Psychosocial Recovery Programs provide personalised one-on-one support focused on mental health recovery, rehabilitation, and building confidence for everyday life in the community.

About the program

Individual Psychosocial Recovery Programs offer tailored support for people experiencing mental health challenges who want to strengthen their skills, confidence, and independence.

These programs are designed for individuals already receiving support from Community Mental Health Services who would benefit from additional psychosocial recovery support as they work towards independent living in the community.

Support is delivered one-on-one, in the comfort of your own home and local community. It is flexible and matched to your goals, with the level of assistance adjusted as your needs change.

What Support is Provided?

The support you receive is based on your goals and what matters to you. This may include:

  • Establishing routines for home upkeep, financial management, and budgeting
  • Investigating and applying for suitable housing options
  • Support with transitioning to new living spaces
  • Building skills for day-to-day independent living
  • Setting up a structured daily schedule
  • Planning meals, cooking, and grocery shopping
  • Building confidence to engage in social and recreational activities
  • Exploring education and vocational training opportunities that may lead to volunteering or employment
  • Building confidence using public transport
  • Supporting a healthy lifestyle

Support packages available

Two Mental Health NGO programs are available across South Australia for adults aged 16 to 64:

Intensive Crisis Support (ICS)

Up to 6 weeks of intensive support.

Non-Acute Psychosocial Support (NAPS)

Up to 4 months of support (up to a maximum of 12 months, subject to approval).

Both programs offer different levels of care – high, moderate, or low – based on the number of hours of support required.

People aged 65 and over may also be eligible if they have been unable to access other suitable programs (such as through the Aged Care system or NDIS).

How to access support

To access this service, a referring body or Community Mental Health Team must complete an application or referral form with the person seeking support.

Once approved, Skylight will assign a Psychosocial Recovery Worker who will provide personalised, individual support in the person’s home and in the community.

The referring body or Community Mental Health Team will coordinate the initial meeting between the person and Skylight. If there is a waiting period, we work to keep this as short as possible.

What happens after a referral?

Once referred and paired with a Psychosocial Recovery Worker, a Care Plan (CP) is created. The Care Plan outlines:

The Care Plan outlines:

  • your goals and the tasks that will support them
  • timelines
  • how often support worker visits will occur
  • clear expectations for everyone involved

Care Plan reviews occur at the midpoint (and more often if needed). These reviews may include the individual, mental health worker, support worker, and family or carers where appropriate. Together, you will check progress, adjust supports, and set new objectives as confidence and community living skills develop.

If another program becomes more suitable, referrals and transition planning can be discussed.

Confidentiality

During the application process, individuals will be asked to sign a confidentiality agreement that allows information to be shared between Community Mental Health Services, Skylight, and other involved care providers. This supports coordinated care.

All parties providing services follow strict confidentiality standards.

This program provides mental health services to rural South Australia.  Skylight’s Country Wellness Connections provides support and mental health services to people living in the Murraylands and Limestone coast.

 

Wellness Connect

Wellness Connect offers one-to-one peer coaching that supports recovery for people living with mental health challenges. Support can focus on receiving practical help with daily living, looking after your health, finding a home, gaining employment.
